New Client Intake Packets
Green Wave now offers several of our intake forms and packet in a downloadable format for your convenience. This means you can choose the intake packets desired to review and fill it out ahead of time making your first visit more efficient. Simply highlight and click the relevant packet to download. Then print it out, fill in the information, and bring it in with you on your first visit.

1. Wellness Evaluation Packet: This packet is to be filled out by those who would like to utilize a personalized wellness plan and attain the discounts. Once the plan is designed other packets may be required.
2. Counseling: We offer several types of counseling services. The packet required is based on the age of the client.
- Youth Counseling Intake: This packet is for children and those younger than 18. If the client is too young to fill the information on their own, than the parent can fill in the information.
- Adult/Couple/Family Counseling Intake: This packet is for any of the adult modes of counseling. Each adult involved needs to fill out their own intake packet.
3. Chiropractic Care: There are two basic types of packets depending on the type of patient.
- General Chiropractic Intake: This is for any type of client other than auto accident.
- PIP Chiropractic Intake: This packet is the only packet required for auto accident patients.
4. Massage Therapy Intake: This packet is for any type of client wishing to receive a massage. (If you wish to use insurance and you do not have a prescription for massage than you need to also fill out a general Chiropractic Intake packet to determine if a prescription can be written.)
5. Organic Facial and Skin Treatment intake: This packet is for those desiring any form of skin care.
6. Weight Loss (Three Packets):
(One for the free seminar and two for the optional evaluation session)
- Toxicity Questionnaire: To be filled out and brought to the Free Seminar.
- Weight Loss Evaluation: May bring to the free seminar, this is needed for the evaluation session:
- General Weight Loss Patent History: May bring to the free seminar, this is needed for the evaluation session. Please use the General Chiropractic intake packet:
